2025-2026 Course Request Forms
All forms must be completed by the student, using their school-provided Google Account. Parents, please do not request access to these forms, your request will not be approved.

If you need to make adjustments to your course requests that were emailed to you on Monday, you can do that using this form:

Updates must be made on your school Google Account, and must be submitted by May 9th.
If you did not receive the email, or want to check your requests again, you can do so using this form:

After completing the form, you will receive an email with the requests entered for you in Skyward. Remember, these are requests, and NOT your schedule for next year. All requests are filled based on student need, course availability, and seniority.
If you have not completed your course requests forms yet, you may do so at the links below. This must be done using the student OCPS Google Account so that we can connect the requests to the student. Please do not request access to the form, your request will not be answered.
